The information we collect and how
At SMC we take privacy seriously and only use the information we collect to provide our services. We do not share the personal information we collect for any other purpose than providing the best possible service for our clients. Non-personal numerical data will be collected for audit purposes to enhance best practice. At any time, you may request a copy of the information we have recorded about you. As a matter of course, we will delete your identifiable information 8 years after your last consultation.
For transparency, listed are the business services we provide and how each service uses the information we collect.
We request personal information to run our business effectively. This is data you provide us directly, for example, your name and contact details, the reason for your consultation and your past and present medical history. We will never obtain information about you indirectly from sources outside our business. We store notes on a password protected computer for access only by the clinician consulted with in the clinic. We consider you have provided consent for us to store personally identifiable information about your health based on your receiving services from us. We will write to your General Practitioner (GP) with a summary of your consultation and will either send this by email or mail this by the postal service. This is done with your consent but if we write a prescription for medicine for you then it is essential that we write to your GP to notify them. We will offer you a copy of the letter either by post or by email clarifying with you that emails send from SMC are not encrypted. We will obtain your consent for all of the above.
Appointments are arranged by you contacting St John’s Physiotherapy by phone or email. We consider your having made the appointment as consent to contact you by this method if necessary, but, if you chose, you may opt out at any time.
Appointment ratings and reviews
After visiting us we may ask you to fill out an evaluation form asking you to rate our services and provide feedback. This is optional and will not affect your consultation or care provided.
Marketing
We will not undertake phone, mail or email or SMS marketing to clients.
Data processors and data locations
We use software solutions within our business to advertise and promote the clinic. No personal data is used. These software solutions store and process data in numerous locations outside our business premise e.g. Facebook.
